Does this work on scanned PDFs?

Only if the PDF has a text layer (selectable text). For scanned/image-only PDFs, OCR is required. Use the free text-layer checker to confirm.

Is it safe for GDPR/HIPAA workflows?

It runs locally and produces image-based redactions that destroy underlying text. You still need to review outputs as part of your compliance process.
